Operation Tuli
1. Divide the class into 3 groups (division may be according to group duty).
2. FIRST GROUP:
SCRUB NURSE:
* Prepares the sterile instruments using a sterile paper provided by the staff. The paper lining of the sterile gloves should not be discarded as this will serve as the eye sheet and or the sterile paper for instruments.
* INTRUMENTS: 1 Scissors
2 Curve Clamps
1 Straight Clamp
1 Needle Holder
3 cc Syringe with Needle
Tissue forceps with teeth
1 pc OS
2 Cotton Balls with Betadine on top of another OS
* Removes the patient’s underwear and positions him on the table with a newspaper lining at the buttocks.
* Gets the patient’s name, age and other personal data needed for the case.
* Does the aftercare of instruments before soaking them in a cidex solution.
CIRCULATING NURSE:
* In charge of getting the patient outside the area prior to the circumcision and at the same time directs the patient to the observation area after circumcision.
* Hands over the anesthesia to the surgeon.
* Collects the paper lining of the sterile gloves of the scrub.
* Is the next person to scrub.
3. SECOND GROUP: group responsible for the dispensing of medications as well as in charge of giving health teachings
MEDICATiONS: Amoxicillin 25o mg TID for 7 days OR
Mefenamic Acid 25o mg every 6 hours for pain
FOR BIGGER, HEAVIER AND OLDER PATIENTS:
Amoxicillin 500 mg TID for 1 week
Mefenamic Acid 500 mg every 6 hours for pain
* Ask about recent intake of antibiotics. If there is another antibiotic currently taken or within the last 2 or 3 weeks, refer to the DOCTOR IN CHARGE OF THE TULI.
* Ask about tany allergies to Amoxicillin or any food and drug.
* Writes the patient’s name in a yellow sheet of paper with the name of the scrub nurse and the surgeon and at the same time fills up the logbook of CES.
* Gives 21 capsules of Amoxicillin and 3 tablets of Mefenamic Acid.
* Gives one dose of each medicine before sending the patient home.
* For patients who cannot tolerate tablets, ask a syrup prescription from the doctor in charge.
HEALTH TEACHINGS:
* Sutures used are absorbable and usually takes 1 to 2 weeks of healing.
* Watch for signs of infection: fever, pus and non healing of wound
* Usual course of inflammation is expected after the circumcision (popularly known as KAMATIS).
* Take full course of antibiotics. Pain reliever on PRN basis.
* PATIENT SHOULD NOT SWIM ON THE BEACH as this might cause infection of the wound.
* Instruct watcher to take note of the dressing on the 1st 34 hours. If it is soaked, go immediately to the nearest health center or clinic.
* Dressing may be removed a day after the tuli and the patient may have a bath at home.
* Post-op site should be cleansed with betadine or with a herbal antiseptic substitute in the form of boiled guava leaves.
* Post-op site may be left without a dressing after provided that it is cleansed with an antiseptic on a daily basis.
4. THIRD GROUP:
* In charge with the coming in and going out of patients.
- Calls on the next patient.
- Sees to it that patients are properly lined outside.
- Decongests the area to provide a conducive atmosphere.
* In charge with the observation area.
- Observes the patient for 1O minutes and notes for any bleeding in the post-op site.
* In charge with the preparation of instruments for the next scrub while the circumcision is going on

Diagnosis: Uncircumcised Penis and or Phimosis as the case may be
Anesthesia: Local
Name of Doctor: (Specific doctor who did the operation;
If done by a nurse, place Dr. Antonio Pablo Ramirez’s name)
1. Assembly time is 7 AM at the SPC Gym and students should be in their Type C uniform.
2. Attendance is checked before and after the tuli.
REMINDERS:
*Things to bring:
· 2-3 pairs of Sterile Gloves
· Heavy Merienda or Lunch
· Jotdown Notebook and Pen
· Bandage Scissors
*Bring 2 sets of BP apparatus and stethoscope per class
*Assembly time: 7:oo AM, SPC Gym
*Uniform: Type C
BLS TRAINING GUIDELINES
1. Students are required to attend the BLS training as scheduled. Attendance will be checked. Absences and tardiness will be recorded and will be sanctioned same with your duty days.
2. Reporting time is 8:oo AM-12:oo NN, 1:oo PM-5:oo PM.
3. Wear any shirts with school seal or shirts bearing the name of SPC (shirts used during intramurals, NSTP, RLE t-shirts). Shirts should cover buttocks and abdomen. You may wear jogging pants or any comfortable pants, with comfortable shoes. Strictly no shoes with heels; no sandals.
4. Hair must be fixed with a bun.
5. Each student is required to bring the ff:
* Extra T-shirt
* 2 pcs. Triangular Bandage
* Compress or any clean cloth
* Mat, Malong or Blanket
* 2-3 Gauze Bandage size 4×4
* Alcohol
